If you are new to our practice, please complete the Online Registrationsent to you via email when you set up your appointment OR print and fill out BOTH the Client Intake Form and Waivers, Consent & Cancellation Policy below before your appointment. Printed forms may be emailed to healingsanctuaryexton@gmail.com or brought with you to your appointment.
Only for clients receiving hypnotherapy services: Hypnotherapy Intake Form
*Please note: First sessions, depending on the service you are receiving, may include overall postural assessment, discussion of health history, issues bringing you in, and goals for the session. We consider this intake a very important part of our work with you. It is considered part of the overall time of the session and will necessarily shorten the actual time on the table. These assessments normally do not take longer than 15 minutes.
IMPORTANT: All children under the age of 18 must be accompanied by a parent/legal guardian who must remain present in the room for the duration of the session; NO EXCEPTIONS. We do not provide services for children under age 13.
